A study conducted by the U.S. Department of Health and Human Agency for Healthcare Research and Quality found that 1 of every 10 patients who died within 90 days of surgery did so because of a preventable medical error.
The medical industry has coined a phrase for some of the worst preventable errors - they're called "never events." These are serious and costly errors in medical care that are clearly identifiable, preventable, and result in serious consequences, which should just simply never happen, including preventable post-operative deaths.
Some states have even enacted legislation requiring reporting of such serious medical errors.
